Letters & Forms
Requests for a letter from the doctor must be submitted using a letter request form. These are available at the front desk or by clicking the following link: Letter Request Form (DOCX, 24KB)
The practice endeavours to have all letters and forms completed as efficiently as possible, however, this is not always possible and they cannot be prioritised over clinical or contractual work. Most letters and forms are private work not provided on the NHS. All other clinical and NHS work takes priority. These letters & Forms are done outside of a GP’s normal working day.
There is a fee for most letters and forms (you can ask at reception).
Please note, that due to the guidelines for British passports, a GP at the practice is unable to sign this form. If you require an Irish passport signed, you must book in for an appointment.

Medical Reports & Subject Access Requests
Our practice has decided to outsource our medical reporting work to a processor, iGPR Technologies Limited (“iGPR”) 01527 570 005.
iGPR will be processing your medical report requests, such as Subject Access Requests (SARs) that you submit to us (or that someone acting on your behalf submits to us e.g. Solicitors) and report requests that insurers submit to us under the Access to Medical Records Act 1988.
iGPR is fully compliant with both current data protection legislation and industry and NHS best practice for the management of patient data and keeping it protected. This ensures the practice of good data security and that personal information is handled correctly.
iGPR helps General Practice with data security, speed and efficiency of producing medical reports. They support surgeries with compliance with the data protection legislation by ensuring that patient data that is to be shared through third parties requests such as solicitors and insurers, is effectively protected and encrypted in transit.

Any u=insurance reports received by the Practice electronically will go directly to iGPR and any updates or issues raised will come directly from them. iGPR can be contacted directly with any queries or updates 01527 570 005.
